Habilitation Coordinator/MI Specialist
Role Responsibilities
The Habilitation Coordinator/MI Specialist evaluates persons seeking admission to nursing facilities to determine their eligibility for specialized services for mental illness (MI). The Habilitation Coordinator/MI Specialist serves as the single point of contact for referrals from the Texana Center Behavioral Health (BH) Division for persons receiving services through both the Behavioral Health (BH) Division and the Intellectual Developmental Disabilities (IDD) Division. The Habilitation Coordinator/MI Specialist works under the supervision of the Diversion Coordinator. The Habilitation Coordinator/MI Specialist is responsible for the following:
- Completing PASRR Evaluation Screenings (PE1) and PASRR Evaluations.
- Participating in PASRR planning and monitoring processes for persons qualifying for MI PASRR services.
- Receiving and responding to referrals from the BH Division for persons with a mental illness and a co-occurring IDD condition.
- Coordinating IDD services and resources for qualifying persons referred by the BH Division, to include eligibility evaluations.
- Facilitate collaborative supports and communication across BH and IDD services and supports.
- Adherence to and promotion of the Texana Compliance Policy.
- Travel within 6 county region.
- Work hours: M-F, 8-5 with flexibility based on client need.
Salary
- $21.71 - $26.26/hour
Required Education and/or Certifications, Experience, Skills
- Bachelor degree or advanced degree in social, behavioral or human services field.
- Texas Driver License (in good standing)
Preferred Education and/or Certifications, Experience, Skills
- Experience with EHR and client data base systems.
- 2 years related experience in IDD or MI field.
- 1 year experience in staff supervision.
- 1 year experience in Service Coordination IDD or MI field.
- Strong organizational and computer skills (Microsoft Office and Electronic Health Record system).
Job related Physical Requirements
- Must be able to operate a vehicle
- Must be able to perform SAMA and CPR. Training is provided to employees once hired
- Must be able to perform moderate to heavy lifting (15#- 45#), pushing, stooping, light carrying (15#), repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting
- Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
