What are the responsibilities and job description for the Population Health Consultant position at Texas Health Care PLCC?
Overview of the Role:
Working closely with the Market-based Population Health teams, primary care and specialty providers, networked partners, and Performance Management at Privia Health, the Population Health Consultant will provide ownership of defined Value-Based Program Performance Metrics, care coordination, and support for Privias at-risk patients. You will develop and implement innovative population health initiatives and programs to improve the quality and cost of care, resulting in improved health outcomes for our patients. You will work with providers, care teams, field leaders, and Population Health teammates to make a lasting impact.
Primary Job Duties:
- Work closely with management and physician leadership to set goals and meet them for population health programs.
- Review performance data for the assigned region and work closely with the population health specialist and Population Health Quality Coordinator to create action plans that support organizational goals for performance
- Build collaborative working relationships with care centers and population health specialists
- Prepare patient, provider, and care center level reports from the EMR and various payer portals
- Demonstrate critical thinking to determine the best way to support care centers as needed, given expertise, knowledge, and strategies.
- Works closely with physicians and their assigned Population Health Specialist (Pop).
- Creates staff training on your assigned population health programs and initiatives.
- Providers train the trainer education for pops
- Handles scheduling, agenda, materials, location, facilitating, and minutes of committee/workgroup meetings as needed.
- Facilitates meetings with physicians and staff as necessary.
- Oversees the hiring, onboarding, and continued training for defined population health team members, including but not limited to Care Manager, AWV Nurse, Population Health Quality Coordinator, Population Health Strategist, and/or Population Health Specialist.
Minimum Qualifications:
- Leadership experience required
- Bachelors degree in a related field required
- Three plus years of management experience
