What are the responsibilities and job description for the Population Health Quality Coordinator position at Texas Health Care PLCC?
Overview of the Role:
Working closely with the Market-based Population Health teams, primary care providers (PCPs), networked partners, and Performance Management at Privia Health, the Population Health Quality Coordinator will provide execution on defined Value-Based Program Performance Metrics, care coordination, and support for Privias at-risk patients. Primary work will include patient outreach to provide coordination services, fill gaps in care, and regular review and action on local performance reports and regional action plans. The candidate will have relevant clinical experience, phone experience, educational background or license/certification to support the Population Health team.
Primary Job Duties:
- Conduct patient outreach in accordance with HIPAA and patient privacy guidelines to support quality-related programs and ensure target goals are achieved
- Assist in scheduling PCP and specialist patient appointments
- Prepare patient, provider, and care center level reports from the EMR and various payer portals rams
- Review performance data for assigned region and work closely with population health specialists to create action plans that support organizational goals for performance
- Build collaborative working relationships with care centers and population health specialist
- Demonstrate critical thinking to determine the best way to support care centers as needed given expertise, knowledge, and strategies.
- Answer and resolve business office inbound calls related to population health, care coordination, or other clinical activities
- Make appropriate referrals to internal care team, payer programs, community resources, home care, and disease management programs
Minimum Qualifications:
- Preferred experience with profession-based telephonic or remote communication
- EHR experience preferred
- Must comply with all HIPAA rules and regulations

- MA, EMT, Case Management certification and/or clinical experience preferred
