Job Summary
The Patient Care Coordinator (PCC):
- Creates, maintains, and updates clinician’s schedules.
- Facilitates the provision of in-home care and support services to clients.
- Routinely interacts with clients, family members, contractors, community providers, referral sources, and administrative and clinical staff.
Responsibilities
- The PCC complies with all organizational policies and procedures regarding ethical business practices.
- The PCC accurately schedules shifts and hours by matching clinical staff and/or contractor qualifications and availability to the clients’ needs.
- The PCC acts as point of contact for interactions with staff, contractors, community providers, referral resources and clients to ensure uninterrupted care of client.
- The PCC communicates new assignments and schedule changes to clinical staff as well as caregivers and clients.
- The PCC processes via client care software the data necessary to initiate accurate billing processes.
- The PCC reviews and reacts in a timely manner to daily and weekly reports as assigned.
- The PCC participates in on-call rotation as assigned.
- The PCC works closely with the Intake Department in regards to admissions, recertification, discharges, and insurance authorizations.
- The PCC works in conjunction with Administrator, DON, ADON, and Human Resources to assist in resolution of client needs.
- The PCC may assist in maintaining and processing orders and scheduling for contractors.
- The PCC may assist in taking and processing verbal and written physician orders.
- The PCC may assist in informing the physician and other personnel of changes in the patient’s condition and needs.
- The PCC attends case conferences and all other mandatory meetings.
- The PCC participates in training and in-service programs to enhance their skills and knowledge.
- The PCC may serve on committees and participate in the development of systems to improve care coordination.
- The PCC conducts themselves in a professional manner, both verbally and nonverbal.
- The PCC promotes a positive attitude at all times.
- The PCC performs other duties as assigned or as needed.
Training Requirements
- A minimum of two years related experience.
- Knowledge of scheduling and/or health care preferred.
- Proficiency in word processing and computer skills.
- Training or experience in human relations, customer service, time management, and organizational skills.
Knowledge Requirements
- Must be able to read, write, speak and understand the English language.
- Understanding of HHA basic functions and goals.
- Basic understanding and knowledge of Medicare and Medicaid policy.
- Basic understanding and knowledge of private insurances.
- Able to make independent decisions when circumstances warrant such action
Professional Requirements
- Maintains and ensures HIPAA compliance.
- Must hold a valid Texas Licensed Vocational Nursing License
- Responsible and accountable to the Administrator and COO regarding time availability and performance.
- Maintains good rapport and cooperative working relationships with patients, physicians, referrals sources, regulatory agencies and staff.
- Adheres to company dress code.
- Attends annual review and departmental in-service training.
- The applicant does not have record of a crime, offense or misdemeanor as defined in 97.241 of the Texas Administrative Code, Title 40, and Chapter 97.
- The candidate must adhere to high personal and professional ethical values.
