Position: Case Manager II-III Family & Youth Diversion [1416]
Program: Amarillo, Texas – Family & Youth Diversion
Salary range:
Case Manager II: $43,305.60 Annually
Case Manager III: $46,009.60 - $52,020.80 Annually
Shift available: Full-time
Essential duties and responsibilities:
- Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
- Will conduct crisis response and coordination with youth and families experiencing or who are at risk of experiencing a crisis episode
- Will conduct comprehensive life needs assessments (CANS/NOMS, etc.) taking into account needs as viewed by the child/adolescent, family member(s), school personnel, present service providers, and significant others in a timely manner
- Provides any needed community referrals, and follows up with the family, referral, and community resource to ensure need was met
- Provides skills training using evidence-based curriculums
- Advocates on the behalf of the child/adolescent served so that access to required services are available and that services rendered in a timely fashion and in accordance with the child/adolescent’s abilities and /or situation
- Will provide outreach in community when needed to inform others about Family & Youth Diversion Program
- Will spend 50% of time with families and youth/providing services to families and youth
- Will maintain same day next day documentation requirements and a 95% or higher compliance average for all families served
- Executes prudent action in emergency situations and will help team members when families need crisis response or other diversion-based services
- Performs other essential functions and tasks necessary to carry out the purposes of the position
- Adheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromised
Required qualifications to be successful in the job:
- Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field for QMHP credentialing
- For Case Manager II, minimum one (1) year of experience in behavioral health case management
- For Case Manager III, minimums two (2) years of experience in behavioral health case management
- Graduate work in a related field may be substituted for up to one (1) year of experience
- Must have a valid Texas driver’s license and be insurable under agency vehicle policy
- Must be 18 years of age or older
Preferred qualifications to be successful in the job:
- One (1) year experience working with youth and families
