The Counselor is responsible for gathering data, documenting problems, and establishing a course of treatment. Provides direct addiction treatment services to adult patients and family members.Essential Duties and Responsibilities include the following and other duties as assigned : Provide individual and group counseling to patients assigned to caseload. May provide telehealth counseling as needed.Direct training groups and seminars. Provide therapeutic leadership in group counseling sessions.Function as a liaison with other community-based agencies and law enforcement agencies to ensure continuity of treatment (referrals) and compliance with legal obligations.Complete individual chemical dependency assessment of new patients, ensuring accuracy and completion of all required forms.Develop an individual treatment plan (ITP) for the new patient within the established time frame, verified by the patient’s signature.Update new patient charts within established time frames, ensuring treatment plans reflect current problems / goals.Work with medical staff in securing urine samples for monthly urinalysis and document results in the patient’s chart.Engage in Diversion Control through execution of random bottle recalls.Provide necessary forms to the Medical Director for special requests regarding medications.Ensure patient confidentiality is protected in compliance with regulations.Maintain all assigned patient files in compliance with DHHS regulations and the Policy and Procedure Manual.Complete discharge of a patient from the program within seven (7) days from the date of discharge.Contribute to ongoing assessment and treatment planning through participation in daily reports, case reviews, and treatment teams.Provide patient education relating to chemical dependency and the process of recovery.Offer supportive counseling or crisis intervention services as necessary.Provide education and public information regarding opioid treatment programs.Participate in professional development and maintain professional credentials.Engage in Continuing Quality Improvement and report on assigned CQI monitors.Fill in at other clinics as needed.Job RequirementsJob Standards / Minimum Qualifications : Two-year degree in counseling or a related field, or experience in another field of similar experience, or must be licensed LCDC or LCDC-I. Must have a valid driver’s license or state-issued ID.Physical RequirementsFrequent sitting at a desk or computer while typing, completing paperwork, or meeting with patients.Occasional lifting / carrying of paperwork, files, and office supplies, up to 20 lbs.Occasional push / pull exerting up to 5-10 lbs. force.Occasional kneeling / crouching and / or ability to reach lower file drawers.Mental RequirementsThe position requires constant mental alertness, attention to detail, and a high degree of accuracy in completing all assignments.
