The Palliative Care Service provides specialist palliative care input to patients of Alfred Health. Team members include Consultant Medical staff, Palliative Care Nurse Coordinators, Junior Medical Staff, Grief Counsellors, Music Therapists as well as Administrative support. Palliative Assessment and Treatment Service (PATS) is a dynamic fast paced clinical service, providing outreaching acute palliative care to people referred to Alfred Palliative Care with life limiting illness’s, with the aim of providing symptom relief and improving their quality of life safely in a range of settings.
We Are Looking For Someone Who Is
An exciting opportunity exists for an experienced registered nurse who has acquired the expert knowledge base, complex decision making skills and clinical competencies for expanded practice. The PATS NP is an integral member of the Palliative Care team, who practices autonomously but in collaboration with other health professionals to assess and manage palliative care patients using nursing knowledge and skills. Extensions to practice include prescription of medications and at least one of the following: ordering diagnostic investigations, direct referral to other health care professionals, the ability to admit and discharge patients and/or provision of absence from work certificates.

- Fixed term long service leave cover until 13 July 2026
- Full Time (76 hours per fortnight) 9 day fortnight, Monday to Friday business hours initially, with view to review and adjust according to service demands

- Expert care: Provide comprehensive assessments, diagnoses, and tailored treatment options directly to patients, whether in-person or through Telehealth.
- Collaborative planning: Work with multidisciplinary teams to create personalised care plans that prioritise patient goals and preferences.
- Empowerment through education: Offer health education and support to patients, families, and care staff, ensuring optimal outcomes and understanding of care processes.
- Community connections: Build strong partnerships with Community Palliative Care services, GPs, residential care staff, and community services and families to manage patient care effectively, reducing avoidable hospital admissions.
- Adapt and lead: Evolve your practice to meet the needs of our service and take an active role in professional development and leadership within the Palliative Care team.
- Extensive advanced knowledge of Palliative Care
- Knowledge of research methods and processes, the ability to generate own research, as well as the ability to analyse and interpret existing data
- High level interpersonal and communication skills across a broad range of health professionals
- Ability to work both autonomously and collaboratively
- Demonstrated ability to be self-motivated and innovative
- Capacity for critical reflection
- Evidence of competent utilisation of extensions to advanced nursing practice according to approved Clinical Practice Guidelines in Palliative Care.
- Current Victorian Driver's Licence
- Current endorsement by the Australian Health Practitioner Regulation Agency as a Nurse Practitioner
- Completed a Masters of Nurse Practitioner (or a Masters granted equivalence of qualification by the Australian Health Practitioner Regulation Agency plus an approved Therapeutic Medication Management module)
