Job Summary:
This position involves providing critical support to individuals with I/DD, facilitating access to medical appointments and services, and fostering positive relationships with individuals, their families, and healthcare providers.
Responsibilities:
- Transport individuals to medical appointments and provide support during visits
- Assist in coordinating medical-related appointments and providing medical services as needed
- Develop and maintain relationships with individuals, their families, and healthcare providers
- Collaborate with colleagues to develop and implement person-centered plans
Requirements:
- High school diploma or equivalent required; experience working with individuals with I/DD preferred
- Valid driver's license and reliable transportation
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
