General Description
The Case Manager is responsible for aspects of the Individual Supports departmental operations associated with provision of services to individuals with disabilities and their families. The Case Manager reports directly to and receives weekly and/or as needed support and supervision form the Individual Supports Team Coordinator.
General Qualifications
Bachelor Degree in Human Services or related field preferred
Minimum of two (2) year’s work experience directly related to aforementioned duties and responsibilities preferred
5 years of Human Service (DD/ID) experience in lieu of Bachelor’s degree
Knowledge of community system serving adults with disabilities
Excellent interpersonal and communication (written/verbal) skills
Ability to work with a wide range of constituencies in a diverse community
Ability to manage challenging behaviors
A safe driving record, current driver’s license, and current motor vehicle liability insurance.
Job Responsibilities
· Supervises DSP program staff and oversees their activities.
· Works collaboratively with team members to facilitate individualized person centered supports.
· Routinely works as staff member within the Individual Supports program.
· Provides supervision to DSPs working with assigned case load
· Participate as a member of the Individual Supports team and attends regular review meetings to ensure appropriate direction of services for each individual.
· Assist with the Recruiting, hiring, and supervision of DSP program staff.
· Works with the Coordinator to provide assessment, development and implementation of individual support strategies
· Assess programming in order to modify and develop services to meet identified needs of individuals with assistance of supervisor.
· Works in conjunction with the Individual Supports Team Coordinator to maintain compliance with all federal/state agency requirements including documentation for both ISP meetings and daily and monthly documentation
· Participates in relevant internal and external committees and task forces.
· Manages day to day operations of case load and their staffing requirements.
· Works directly with supervisor and agency nurses to ensure all Health and Wellness Policies and Standards are met.
· Works with individuals, family members, state and other agency representatives to promote and develop natural community based supports and opportunities.
· Participate in the on-call system within the organization and/or as a backup emergency contact person to the Administrator on call.
· Maintains necessary certifications/training
· Other duties as assigned.
