General Description
The Treatment Coordinator is directly responsible for the case management of services to children with developmental and other special health care needs. Responsible for coordination of Home-Based Therapeutic Services (HBTS) and Personal Assistance Service and Supports (PASS) to include planning, quality, and outcome-oriented services, staff supervision, training and scheduling as outlined in each program’s State of Rhode Island Agency Certification Regulations.
The Treatment Coordinator reports directly to and receives weekly and/or as needed, support and supervision from the Treatment Coordination Supervisor. The Treatment Coordinator works in accordance with the policies and procedures of The Arc of Northern Bristol County, Inc d.b.a. proAbility, The State of Rhode Island’s Department of Human Services, and all other funding sources to oversee and coordinate individualized services to children with special healthcare needs and their families.
Minimum Qualifications
Bachelors Degree in related field of study
5 years of experience working with individuals with developmental/intellectual disabilities
3 years of supervisory and/or leadership experience
Experience in advocacy for individuals with disabilities
Safe driving record, current driver’s license, and motor vehicle insurance

Job Responsibilities
1. Protect the rights and dignities of individuals with developmental and other disabilities and extend these rights and dignities to family members or guardians.
2. Uphold and comply with all agency policies and safety programs to maintain a safe work environment for employees, individuals, family members, and visitors. Adhere to all other agency guidelines and department specific policies
3. Comply with State regulations regarding referrals and intakes, implementing proAbility’s Referral and intake policies to meet with and gather information from families in response to a request for services for their child.
4. Facilitate completion of all releases, collect information, work with clinician to write plan (as appropriate), write contact notes, and submit billing with appropriate approvals. Ensure timely submission of all required documentation.
5. Recruit and train staff in accordance with agency and state policies and procedures. In collaboration with the Clinical Supervisor, provide staff training, mentoring, performance evaluation and disciplinary action as appropriate to specific services (HBTS/PASS).
6. Provide quality assurance/oversight to the delivery of services for individuals on caseload.
7. Communicate with all appropriate members of the individual’s team to ensure that services and supports provided meet their needs.
8. Commit to organizational quality initiatives by participating in programs that will.
9. Improvement and team processes, including regular attendance at staff and team meetings.
10. Serve in the on-call rotation within the proAbility service delivery system as directed by supervisor.
11. Represent The Arc of Bristol County/proAbility in the community by representing the agency in a positive and supportive manner within the community and with the individuals and families we support.
12. Supervise assigned direct support staff including training, scheduling, discipline, and performance reviews. Maintain accuracy and timeliness in the review of time taken (sick, vacation, etc.) and submission of supervised staff mileage and timesheets.
13. Meet with staff regarding individual plan(s) of service. Conduct monthly (minimum) home visits with family. Provide shift observations and supervision to direct support staff on a monthly (minimum) basis.
14. Working in conjunction with the Office Manager, ensure proAbility’s children’s services data base remains current. Responsible for accurate and timely completion of data calculations and reports, and timely submission of service plan renewals, progress notes, and all other pertinent documents. Communicate with clinicians regarding all regulatory deadlines for plan renewals, progress notes, etc
15. Maintain Treatment records for all assigned individuals in accordance with agency policies and state regulations.
16. Perform other duties as assigned.
