What are the responsibilities and job description for the Pre-Employment Transition Coordinator position at The Arc of Greater Boone County?
The Pre-ETS (Pre-Employment Transition Services) Coordinator will be responsible for services at Sheridan and Hamilton Heights school systems with students who have IEPs to provide coordination and services for Transition Teams. The Pre-ETS Coordinator is there to ensure student outcomes including employment, life skills and post-secondary education as determined by the individual student’s interests, talents and skills. The Pre-ETS Coordinator will assist the Transition teams to work with the individual, their family and any additional support team members in the following ways:
- Conduct programs of responsibility in a safe and health conscious manner.
- Provide development, implementation and coordination of a transition team in each assigned school.
- Provide or assist school teams with direct services to individual students, including but not limited to: Job exploration, Work based learning experiences, Workplace readiness training, Self-advocacy, Counseling on opportunities for post-secondary education
- Support/assist school personnel, families and students to access additional supports including but not limited to: Medicaid Waiver application completion, filing of documents with BDDS office and follow up, Vocational Rehabilitation referral, consultation and follow up, Guardianship and alternative to guardianship resources, Benefit analysis recommendation, how working affects SSI/SSDI benefits
- Participate in and create various community collaborations. (Chambers, Kiwanis, Rotary, committees)
- Build and maintain relationships with employers in committees to use for discovery and possible employment, volunteer opportunities.
- Prepare paperwork, schedule and participate in monthly Vocational Rehabilitation requirements.
- Track and maintain assigned project outcome data and submit to supervisory staff, State, and Project Manager on a monthly basis.
- Maintain training data required by CARF and Vocational Rehabilitation such as task analysis, etc. for each individual student enrolled in the grant project.
- Connect individuals to relationships and support in the community.
- Coordinate and communicate effectively with the School and Project staff throughout the Individual Service Plan process.
- Conduct Needs Assessments as needed.
- Gather information regarding eligible students in new schools and each new school year.
- Identify and assist students and families who may be considered “gap” students. This is in reference to students who have the greatest barriers and are not considered the most significantly disabled. These students would be eligible or may be eligible for Vocational Rehabilitation services and are in need of support to succeed.
- Provide weekly schedule and updates to the Career Connections Manager.
- Provide individual and small group discussion centered on employment or post-secondary education.
- Assist with training and oversight of new personnel when appropriate.
- Other duties as assigned.
Requirements: Minimum of Bachelor’s Degree in Rehabilitation, Education or related field or High School Diploma with 4 years’ experience in rehabilitation working with students/individuals with disabilities. Valid Driver's License with reliable transportation and car insurance. Ability to work occasional weekend hours.
