JobPurpose
The Program Coordinatorguides Direct Support Professionals for the Community Integrationteam. They ensure participants actively engage in activities in ourcommunity centers and the local community.
The Program Coordinatorensures alignment between our program and the individualparticipants comprehensive plan of care (CPOC).
Essential JobFunctions
Ensures compliance with relevantstandards and policies (regional state federal CARF) for licensingand certification as they relate to the Cldepartment
Communicates withindividuals families guardians case management and funding agenciesto advocate on behalf of theparticipant
Plans andoversees the implementation of center and communitybasedactivities
Produces andmaintains current records as it pertains to participants activitiesetc.; assuring compliance with all applicablestandards
Directlysupervises all direct support personnel at their specifiedcenter.
Ensures facilitymaintenance is prioritized and promptly reports needed repairs tothe Facilities Manager
Mustensure that the ratio of their center is properly maintained whenexpected to attend scheduledmeetings
Ensures timelysubmission of requested reports (block reports safety reportsmonthlies weekliesetc.)
Assures meaningfulfunctional and ageappropriate activities are offered for alldesignatedparticipants.
Monitorsprogramming personnel and makes recommendations to improve servicesbeing provided
Activelyparticipate in planning meetings for participants (quarterly / annualmeetings) by providing information about participant engagementlikes dislikes goals etc. atmeetings
Assumes primaryresponsibility for addressing all participants family and directsupport staffgrievances

Education / ExperienceRequirements
Bachelorsdegree in human services or related discipline or three (3) yearsof equivalent work experience in a related fieldpreferred
Excellent writtenverbal communication and organizational skills and demonstratesprofessionalism in allrelationships
Possesses theability and willingness to maintain a flexible work schedule(evenings and weekends as consumer / staff needsindicate.)
Possesses basiccomputer skills and a willingness tolearn
Protectsconfidentiality of program participants parents / advocates staff anddepartmental businessaffairs
Maintains validDrivers License current auto insurance and a vehicle in goodstanding according to the ArcGNO VehicleInspection.
WorkingConditions & PhysicalDemands
Thisrole is subject to a moderate level of physical activity includingbut not limited to sitting standing walking reaching or carryingitems as needed
Occasionallylifting 50 pounds
Frequentlylifting and / or carrying objects up to 20pounds
Unrestricted walkingandstanding
