JOB TITLE : Program Coordinator
REPORTS TO : Program Manager
FSLA CLASSIFICATION : Exempt
SALARY : $50,000
JOB PURPOSE :
Under the supervision of the Program Manager the Program Coordinator will have responsibility for carrying out the day-to-day functions of their assigned home. This includes securing the well-being and safety of individuals with disabilities which requires; assisting individuals with personal care and daily living skills; training and supervision of Direct Support staff; maintaining house / site facilities; and a broad range of program wide administrative tasks within designated area.
This position requires a commitment to individual choice and community integration and inclusion. Travel by car is necessary and a great deal of time will be spent in individual houses / sites. This position requires the ability to work where needed within the Tri-County area. In addition, responding to emergencies during and after hours, on weekends, and holidays is required when applicable.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
SUPERVISING DIRECT SUPPORT PROFESSIONAL (DSP)
- Schedule planning and review
- Maintaining adequate staffing ratios
- Approve leave / timecard review and submit to Program Manager for review
- Fill in for staff when needed to cover the shifts
- Assist the Program Manager with interviewing and hiring of potential direct support staff
- Provides individualized orientation to new staff members, mentors and provides appropriate / constructive feedback, when applicable
- Assures adherence to policies and procedures through training feedback
- Delegating and assigning duties to DSPs
- Ensuring review / signatures of all plans (nursing, behavior, etc.)
- Monitoring electronic health record for case notes and documentation (client dashboard)
- All assigned tasks are completed daily and logged accurately
- Complete house inspections / fire drills and monthly attendance
- Coordinate / facilitate house meetings
