The Arc San Francisco is a non-profit service and advocacy organization for adults with autism, Down syndrome, cerebral palsy, intellectual and developmental disabilities, and their families living in San Francisco and San Mateo counties. We currently serve a varied group of over 550 clients who, despite their diverse backgrounds, ages, and disabilities, are all similar in their desire to live a life of greater self-determination, dignity, and quality. Our goal is to provide support that enables people with developmental disabilities to maximize their potential, live meaningful lives and become integral members of the community. To that end, we provide a range of services relevant to all areas of adult life, from independent living supports and skills building to employment training and creative expression. Position Summary
Serves as a key agency resource with a dual focus on Coordinated Family Supports (CFS) and Health Advocacy. Delivers individualized education and training to participants, families, and service providers.
This role is distinguished from the Health Advocate position by the inclusion of CFS participants, and its advanced requirements including a deep understanding of community resources, proficiency in assessment tools, content development, training delivery, data analysis, and database management.
Essential Duties And Responsibilities
Key Competencies: To perform the job successfully, an individual should demonstrate the ability to exhibit the following competencies:
Repetitive keyboard use, phone use, walking, standing, bending stooping and direct client interaction. Ability to lift wheelchairs and walkers into cars. Ability to assist clients with transferring in and out of cars from wheelchairs.

Essential Duties And Responsibilities
- Responsible for a dual caseload of Coordinated Family Supports (CFS) and Health Advocacy participants.
- Meet with CFS participants and their families at home to assess needs, barriers to supports, and identify helpful solutions, using a person-centered approach.
- Help participants, families, and service providers set and achieve CFS goals.
- Provide tailored education and training to ensure all of CFS participants’ providers deliver services consistently and meet specific needs. Provide training to community members as needed.
- Find and coordinate backup support options for CFS participants, offering temporary help until permanent solutions are in place.
- Connect families with community resources like CalFresh, Meals on Wheels, exercise classes, support groups, and transportation, and help them overcome any barriers to access.
- Meets with participants and circles of support to complete Health Advocacy assessments.
- Provide Health Advocacy services including but not limited to: helping participants manage health and access medical appointments, online medical records, and other health resources.
- Keep an updated database of community resources that understand the needs of individuals with IDD and reflect their languages and cultures.
- Offer future planning services for participants, especially those living with aging parents.
- Work with Associate Directors and Service Managers to identify data needs, track data, and analyze trends to improve programs.
- Complete required paperwork and reports.
- Manage monthly billing and ensure compliance with usage levels.
- Perform other assigned tasks as needed.

- Knowledge: Possesses technical knowledge required to do the job including knowledge of public health and/or working the developmentally disabled population. Basic medical knowledge, including the ability to recognize social and medical risk factors, and chronic disease conditions. Knowledge of San Francisco community resources, Medicare, and Medi-Cal.
- Judgment: Exhibits sound and accurate judgment; can clearly explain reasoning for decisions; includes appropriate people in decision-making process; demonstrates persistence and overcomes obstacles
- Customer Service: Ability to work comfortably with diverse populations, with sensitivity to issues concerning developmental and all disabilities. Responds efficiently and cordially to requests for service and assistance interdepartmentally
- Professionalism: Reacts well under pressure; treats others with respect and consideration; follows through on commitments
- Innovation: Displays original thinking and creativity; meets challenges with resourcefulness; develops innovative approaches and ideas
- Associates degree in a human services field of study or at least three years of experience working with individuals with developmental disabilities. Health Advocacy and/or familiarity with community resources preferred.
- Valid California Driver’s license, safe driving record and acceptance as a covered driver by The Arc’s auto insurance company.
- Ability to operate a motor vehicle, use public transportation, and use ride transportation services.
