About the position
Success for the full-time Care Manager position looks like guiding adults and/or children with mental health with kindness and empathy to improve their overall life's experiences. You will provide comprehensive care management through engaging with your clients to fully understand their situation and then you will develop and follow through on a case management plan that will truly make a positive difference in their lives.
Work location- The Arc Wayne, 150 Van Buren St, Newark, NY 14513 (Will require travel to off-site locations)
Work schedule- Monday through Friday (8am to 4pm) with flexibility to meet agency's needs.

You should have
- A Bachelor's Degree in a Human Services field or related field or NYS licensed Registered Nurse
- Two years of experience providing direct services to person diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse.
- Computer experience with current working knowledge of windows, Microsoft programs and Electronic record/documentation software
- A current and valid New York State driver's license.
