Program Description :

The Philadelphia Model : Addressing Serious Mental Illness (SMI) in Long-Term Care (LTC) will provide highly structured therapeutic residential behavioral healthcare and treatment for frail adults who are eligible for skilled nursing level of care who also have serious mental illness and who have reached maximum benefit from the mental health resources available elsewhere in the community or hospital. The program philosophy is to create a welcoming, safe, and supportive environment where frail older adults with SMI that may include other complex behavioral health needs and / or prior criminal history – and need long-term skilled nursing services – can receive the care and services needed to address their holistic needs free of stigma. A multi-disciplinary team will provide both long-term care and behavioral health services in this integrated program. All team members will adhere to the Be Well at MPAC philosophy of Diversity, Equity, and Inclusion and adhere to and exhibit the Be Well at MPAC values of Choice, Hope, Integrity, Respect, and Professionalism.

Location : This role is based at 4001 Ford Road, Philadelphia, PA 19131.

Job Overview :

The Behavioral Health Technician delivers direct, high-quality, and cost-effective behavioral health care to residents, working under the guidance of the Program Director and Licensed Nurses. This role involves collaboration with other licensed professionals, such as social workers and program psychiatrists, to ensure comprehensive support.

Job Responsibilities :

Develops and implements behavioral health coaching to enhance residents' daily lives, addressing various aspects such as relationship and community support, social interactions, nutrition, environmental factors, and religious and cultural considerations, all under the guidance of a Licensed Nurse.

Aids CNAs in the daily personal care tasks, including assistance with feeding, morning and evening routines, and facilitating communication.

Observe residents' health status and help administer therapeutic care, ensuring adherence to medication schedules.

Promotes healthy and socially appropriate behaviors within the community setting.

Encourages positive behaviors to maintain boundaries with fellow residents, staff, and visitors, stepping in as necessary to uphold a safe environment.

Assists residents in achieving their clinical objectives as specified in their individualized care plans developed by the resident and treatment team.

Offers de-escalation strategies and trauma-informed interactions as required in response to behavioral issues.

Engages in calming residents during tense situations and initiates emergency protocols when needed.

Responds to emergencies, documenting resident behavior before and after the incident.

Accompanies residents to interdisciplinary care meetings and provides support during medical, legal, and recreational activities and appointments.

Additional tasks as assigned.

Qualifications :

Must meet all Community Behavioral Health (CBH) education and experience guidelines – bachelor's degree in human services OR HS diploma / GED equivalent and 2 years experience (paid or unpaid) providing direct care in a human services field required.

12 College credits in human services are strongly preferred

Must have 3-5 years’ experience providing care to older adults.

Current CPR certification is required.

Previous Psychiatric or Addiction experience preferred.

Successful and satisfactory completion of facility orientation and a Behavioral Health Technician curriculum including feeding certification, HIV, TB, STD, and De-escalation – all required for continued employment.

Bilingual capabilities (English / Spanish) are a plus
