Summary
The Case Coordinator is a part time role responsible for coordinating and overseeing the comprehensive care and support services for individuals or families facing various challenges. The primary goal will be to assess needs, develop care plans, and connect clients with appropriate resources and services to ensure their well-being and progression towards their goals. Responsibilities include providing administrative assistance including food distribution, maintaining records, and organizing documents.
Responsibilities
Client Assessment
- Conduct comprehensive assessments of clients’ needs and develop individualized care plans.
- Identify risk factors, barriers, strengths, and support systems.
Care Planning
- Develop individualized care plans with clear goals and timelines.
- Collaborate with clients, families, and interdisciplinary teams to create and adjust plans.
- Provide ongoing case management services, including regular follow-ups and reassessments.
- Conduct home visits and accompany clients to appointments, as necessary.
Resource Coordination
- Collaborate with clients, their families, and relevant stakeholders to identify and access appropriate resources and services.
Advocacy
- Advocate for clients' rights and access to services.
- Assist clients in navigating complex social service systems.
Monitoring & Evaluation
- Monitor clients' progress and modify plans as needed.
- Maintain detailed case notes and ensure compliance with organizational and legal standards.
Collaboration & Communication
- Stay informed about community resources, programs, and services that can benefit clients.
- Work closely with social workers, medical professionals, and service providers.
- Collaborate with internal teams and external agencies to facilitate seamless service delivery.
- Attend regular staff meetings, training sessions, and professional development opportunities to enhance knowledge and skills.
Crisis Intervention
- Provide crisis intervention and support during emergencies or critical situations..
- Develop safety plans and emergency responses.
Food Security and Special Projects
- Assisting with Food Pantry operations
- Other duties, as assigned
Requirements
Eligibility
- Bachelor’s degree in social work, psychology, counseling, or a related field. A combination of formal coursework and experience will be considered.
- Proven experience in case management, social work, or a related field.
- Knowledge of community resources and social service programs.
- Pre-employment drug screening
- Valid driver's license
- Health Statement
- Must be a U.S. Citizen
- Must pass a criminal background check
Essential Skills
- Excellent communication skills both verbal and written, with the ability to effectively interact with diverse populations.
- Strong problem-solving and decision-making abilities.
- The ability to work with and collaborate effectively with other team members.
- The ability to maintain consistent office hours of at least 5 hours per day, 5 days a week.
- Proficient computer skills including Google/Microsoft Suites and Monday.com
