Essential Position Functions:
- Supervise and coordinate the delivery and documentation of services to adults with histories of homelessness, serious mental illness, and/or substance use disorders, who are living in independent apartments within the community.
- Carry a caseload as needed.
- Ensure case records are current and accurate, including monthly progress notes, collateral contact notes, support plans, assessments, and consents.
- Conduct apartment inspections across multiple floors in the community, utilizing stairs.
- Supervise case managers and their caseloads.
- Train staff to provide culturally sensitive, client-centered support services in the field.
- Assess and assist in crisis situations as they arise.
- Prepare programs for audits and site visits by funding and regulatory agencies.
- Provide education to clients, including assistance with benefits, completing housing applications, connecting to primary and behavioral health care, and providing housing referrals.
- Be available on-call for 24-hour crisis intervention, rotating with other supervisory staff.
Qualifications
Required Knowledge, Skills, and Abilities:
- Bachelor’s degree and at least 2 years of relevant experience.
- Experience working with individuals who have histories of homelessness and/or serious mental health diagnoses, including crisis intervention and managing trauma-triggered behavior.
- Excellent time management skills and stress resilience.
- Flexible, team-oriented, and open to feedback.
- Creative with excellent organizational skills.
- Willingness to perform CPR/First Aid/AED/CPI/Naloxone.
- Preferred knowledge of AWARDS.
- Preferred supervisory experience.
