Job Description:
The Center is seeking Case Managers to work with adults in our Community Support Services Program, with the following job duties:
- Provide one-on-one support to assist adult consumers in daily living skills including but not limited to nutrition, finance, personal care, making and keeping necessary appointments, and symptom management.
- Assess and monitor consumers’ progress towards treatment plan goals by providing behavioral health interventions.
- Work with clinicians and treatment team members to understand consumer's mental health needs and assist in developing treatment plans for assigned clients.
- Assess and monitor consumers eligibility for qualified services.
- Participate in agency training and ongoing professional development as required.
- Accurately complete all required documentation including client data, progress notes and service specific information within required time frame for completion
- Facilitate travel/transportation to conduct consumer visits in home or in the community as outlined in the treatment plan and within the context of the provided service.
- Provide Crisis and on-call staffing as scheduled and/or required
Requirements Include:
- Background void of issues related to abuse or neglect
- Valid Kansas driver’s license with favorable record
- Bachelor's degree is preferred; or 4 years of equivalent work experience
Helpful experience
- Knowledge of social service resources
- Previous experience along with a knowledge of general recovery concepts
- Basic computer skills and knowledge
