Responsible for coordinating and overseeing all aspects of the Agency’s quality assurance program, patient medical record, and infection control program. Other areas of responsibility include supervision of employees within the QA Department.
- Accountable for the coordination and completion of quarterly chart audits.
- Oversees the audit process for regulatory and Agency compliance.
- Identifies problematic areas, evaluates, and implements resolve through the QAPI process.
- Assists the Documentation Nurse as needed.
- Responsible for OBQI and OBQM reports.
- Participates in the nursing leadership on call rotation schedule related to clinical issues.
Medical Records:
- Ensures accuracy of content of patient medical records.
- Obtains information for regulatory surveys and ADR requests.
- Supports the Agency through any State and Federal Survey processes.
- Maintains accuracy and revisions of clinical forms as necessary.
- Responsible for and ensures OASIS compliance.
- Oversees the review, correction, and transmission of OASIS information.
- Coordinates storage and destruction of confidential documents.
Infection Control:
- Responsible for maintaining and tracking employee exposure occurrences, and ensuring appropriate employee health and OSHA records are maintained.
- Responsible for review and update of the Exposure Control Plan at least annually, and whenever necessary to include new or modified tasks and procedures.
- Facilitates Agency infection control efforts through the Infection Control and QAPI processes.
Supervision:
- Participates in the selection, hiring, and termination of staff.
- Demonstrates support for Agency policies and ensures staff compliance.
- Maintains knowledge on state and federal rules and regulations.
- Conducts department meetings.
- Supervises employees by maintaining performance notes, giving required performance evaluations on a timely basis, and initiating progressive disciplinary action as needed.
QUALIFICATIONS:
- Maintains current RN licensure in Indiana.
- At least 5 years of experience in a healthcare setting, with a focus on quality assurance or quality improvement.
- Hospice Experience: Specific experience in hospice care
- Highly Skilled: Microsoft office, EMR’s Excel, Power Points, and Teams
- Analytical Skills: Proficiency in data analysis and performance improvement.
- Regulatory Knowledge: Understanding of hospice regulations and quality standards.
- Quality Certification: Certifications such as Certified Professional in Healthcare Quality (CPHQ) can be beneficial.
- Communication Skills: Strong verbal and written communication abilities.
- Compliance: Ensuring compliance with regulatory requirements.
- Training: Providing training and guidance to staff on quality assurance initiatives
