Job Overview:
The Nurse Liaison for a Skilled Nursing Facility (SNF) serves as the key point of contact between the facility and hospitals, physicians, patients, families, and other healthcare providers. This role is responsible for ensuring smooth patient transitions, providing education, advocating for patients' needs, and maintaining positive relationships with the community and healthcare network.
Key Responsibilities:
Patient Care Coordination:
- Serve as the liaison between the hospital, physicians, and SNF staff to ensure smooth admissions, discharges, and transitions for patients.
- Ensure timely, efficient communication between all parties involved in patient care, including patient updates, concerns, and changes in status.
- Collaborate with the interdisciplinary care team to develop individualized care plans for patients.
Clinical Support & Education:
- Provide educational support and guidance to patients, families, and staff regarding clinical services, facility policies, and the overall care process.
- Educate referring physicians, hospital discharge planners, and case managers about SNF services and capabilities.
- Conduct in-service training sessions for staff to ensure optimal care delivery and up-to-date best practices.
Patient Advocacy:
- Advocate for the needs and preferences of patients to ensure high-quality, compassionate care throughout their stay at the SNF.
- Address and resolve patient or family concerns regarding care, treatment plans, or facility services.
- Ensure that patient rights and confidentiality are maintained at all times.
Quality Improvement:
- Monitor and track patient outcomes, quality measures, and satisfaction, contributing to continuous improvement efforts.
- Assist with the development of care protocols and implement initiatives to enhance the quality of care and facility operations.
- Participate in facility meetings to review trends, feedback, and potential improvements in patient care.
Relationship Building:
- Establish and maintain strong, professional relationships with referring hospitals, physicians, case managers, and discharge planners to facilitate referrals and build trust within the healthcare community.
- Participate in community events and networking activities to promote the facility’s services and foster positive relations.
Documentation & Reporting:
- Maintain accurate, up-to-date patient records, admissions, discharge summaries, and communication logs.
- Provide regular reports to SNF management on patient admissions, status changes, referrals, and any other relevant clinical data.
Required Qualifications:
- Registered Nurse (RN) licensure in the state of New Jersey
- Minimum of 3 years of experience in nursing, with at least 1 year in a skilled nursing facility, hospital, or long-term care setting.
- Strong clinical knowledge and experience in managing patient care in a long-term or Skilled Nursing Facility
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of an interdisciplinary team.
- Ability to prioritize, handle multiple tasks, and meet deadlines.
- Knowledge of healthcare regulations and quality standards.
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN).
- Experience in patient advocacy or care coordination.
- Previous experience working in a liaison role or with hospital discharge planning teams.
- Familiarity with electronic health records (EHR) and patient management systems.
Physical Requirements:
- Ability to lift, move, or assist patients as necessary.
- Ability to stand, walk, or sit for extended periods.
