Position Summary:
The Unaccompanied Alien Children Program referred by the Office of Refugee Resettlement (ORR), provides temporary residential care and other supportive services to Unaccompanied Alien Children (UAC) who are under legal custody of the Federal Government. LPN professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. The LPN will provide comprehensive assessments of physical, mental and psycho-social needs. The LPN must also adhere to policy and procedures as well as help the medical coordinator develop care plans to provide effective outcomes. The Medical Assistant is to provide coverage when the medical coordinator is off; including weekends, assist in all aspects of health management as required by the ORR program and aide in all reporting requirements.
Duties/Responsibilities:
- Assist with initial medical assessments
- Communicate with LPN & RN to assist in the referrals of youth to appropriate medical specialties
- Assist with required documentation for prescriptions
- Assist with follow-up services for UAC programs
- Administer whisper test, vitals, & vision test
- Provide support with creating and electronically submitting TARs for each UC as needed and required paperwork.
- Create and update information for each UC in the UAC Portal electronic records.
- Properly document each medical service.
- Work collaboratively with the LPN, RN, Case Manager, Clinician, and Cottage Manger
- Provide appropriate services to each youth
- Responsible for all medical reporting requirements
- Responsible for documentation preparation
- Responsible for ordering medical supplies
- All other duties as assigned
- Ability to speak, read, and writes English & Spanish fluently, which allows full communication with agency staff and 3rd parties.
- Working knowledge of computer systems, spreadsheet, and word processing software (Excel and Word).
- Possess a valid NYS driver license with a driving record acceptable to CHK’s insurance carrier and possess the ability to drive agency vehicles to other agency sites and places for training purposes.
- Pediatric, psychiatric experience preferred
- Ability to work effectively in a team context
Qualifications:
- Current Valid LPN license required
- At least 21 years of age
- Hold current American Red Cross First Aid & CPR certification
- Associate’s Degree Required; Bachelor’s Preferred in Nursing
- Experience working in congregate care facility.

- Varicella: two doses of varicella vaccine (at least four weeks apart) OR born in the United States before 1980 with a reported history of chickenpox (verified by an adult present at the time of illness) OR titers indicating varicella immunity OR documentation from a healthcare provider verifying previous infection.
- MMR: two doses of MMR vaccine (at least four weeks apart) OR titers indicating immunity to all three diseases.
- Tdap: one Tdap dose.
- Influenza: one dose annually during flu season. Per the CDC, flu season is from October through May.
