Responsibilities:
The Developmental Consultant Liaison is a two-part role.
When following Hospital Liaison role:
Conducts assessment screening to newly delivered mothers and babies in assigned hospital location(s) to determine eligibility status for Healthy Beginnings referral services.
Collaborates with assigned hospital personnel to ensure screening assessments are conducted.
Serves as liaison between hospitals, supervisors, program participants, and collaborating partners.
Assists eligible program participants to access program services, referrals and linkages.
Completes all necessary paperwork in a timely manner relating to the administration of screens and assessments, partnering agency collaborations, collection of program data, and eligibility requirements.
Ensures timely input of program data into designated databases to ensure program validity.
When following Developmental Consultant role:
Provides Screening and Assessment services for children through age five (5) and/or their families utilizing approved assessment tools.
Analyzes and interprets screening and assessment data; identifies and recommends appropriate services to system participants.
Effectively engages clients and serves as liaison between program participants and collaborating partners.
Conduct DC visits with families in their homes or other designated locations within the community.
Ensures flexibility and accessibility to families by accommodating appointments after school, in the evenings and/or on the weekends when families may be more available.
Records and reports appointments, referrals, home visits, and all other work related Healthy Beginnings Program activities to supervisor on a daily basis
Qualifications:
Bachelors degree in Human Services (preferably in a field such as social work, developmental psychology, and/or early childhood education).
Minimum of two (2) years experience in child welfare, primary prevention services, child development or related field.
Bilingual English/Spanish required.
Valid Floridas drivers license is required.
