Position Summary:
As a Case Manager, you’ll be at the heart of our person-centered planning process, working with individuals with intellectual disabilities, developmental disabilities, or brain injuries. You will support these individuals in achieving their goals by coordinating access to medical, social, educational, and community services.
Essential Functions:
- Conduct person-centered planning and assessments to help individuals understand their goals and needs.
- Facilitate participation in assessments and the person-centered planning process.
- Assist individuals in identifying preferences, strengths, goals, and needs to enhance their planning.
- Work with support teams and service providers to ensure access to appropriate services.
- Support the modification of rights and freedoms as needed, ensuring compliance with federal Medicaid HCBS requirements.
- Manage service provider matching and work with the State for service authorization.
Position Type/Expected Hours of Work:
This is a full-time position, Monday through Friday, 8:00 am – 5:00 pm. Occasional evening and weekend work may be required.
Travel Requirements:
Local travel during business hours is required, with some occasional out-of-area and overnight travel.
Required Experience:
- Each Case Manager must have knowledge of Vermont’s public service system for developmental disabilities services and brain injury services.
- Bachelor’s degree in any field and at least one year of experience in human services (e.g., human services related experience, such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work or another related field,) OR
- Associate’s degree in human services or a related field and at least two years of experience in human services (e.g., human services related experience, such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work or another related field).
- Must reside in Vermont.
