JOB SUMMARY
The Care Management Specialist provides needs assessments to clients, and performs duties relating to case management including making home visits, assisting clients in obtaining supportive services, making necessary referrals, providing training regarding independent living skills, attending court hearings advocating for future and current clients within the justice system.
ESSENTIAL FUNCTIONS
Essential functions are duties which are essential or primary to the position or the reason the position exists. An individual must be able to perform the essential functions of the position with or without reasonable accommodation. The Care Management Specialist performs an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties:
1. Assesses needs of client, assists in development of treatment plan, and gathers information relevant to admission.
2. Assists in obtaining supportive services to aid the client in their personal goals in recovery through the services of the agency or through referral to other agencies or programs. Assist clients in accessing community resources and provides supportive services as appropriate.
3. Provides group educational services in an effort to improve coping skills and provide a forum in which to share and learn communication skills.
4. Provides written documentation of client activity/treatment plans and periodic written updates in order to maintain compliance with requirement/standards and provide documentation for billing of services. Initiates all correspondence and paperwork related to needs assessments and client service reports (i.e., letters, progress notes, rosters, mailing lists).
5. Arranges after-care services for clients after completion of treatment. Maintains contact with client to follow-up on progress and to ensure client is complying with treatment requirements.
6. Administers drug screening tests, and other screening instruments.
7. Coordinating with courts, probation officers and other community resources.
8. Providing clients and family members with information about addiction issues and available services/programs.
9. Outreach to referral sources to assist in development of agency to agency relationship and advocacy for clients.
SECONDARY FUNCTIONS
Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc.
7. Assists in office tasks when needed. CASE MANAGER 2
8. Provides input to treatment team and communities regarding the developments and needs of the client population. Meets with clinical staff and Treatment Director for review, input and consultation.
9. Performs other duties as assigned by the General Manager.
COMPETENCIES
1. Demonstrated knowledge of alcoholism and chemical dependency.
2. Demonstrates understanding of clinical practice and performance including understanding of the process of assessment, treatment, and confidentiality.
3. Ability in the area of effective communication, interpersonal skills.
4. Ability to complete all documentation in accordance with agency policy.
5. Effective time management/organizational skills.
MINIMUM QUALIFICATIONS, INCLUDING TRAINING AND EXPERIENCE
1. High School Diploma or equivalent, Behavioral Science Degree preferred.
2. Knowledge of alcoholism and other addictions, experience with 12-Step Recovery Program preferred.
3. Valid Ohio Drivers License and ability to maintain insurability preferred.
4. Maintain CPR/First Aid treatment preferred.
5. Maintains applicable agency trainings.
