Title : CMA-Care Coordinator Pay $20-24 / HR Hours : Full Time Location : Concord, CA Responsibilities :
- Creates, reviews, and revises patients plan of care.
- Documenting patient labs, blood pressure, blood sugar, weight, diet / nutrition, and any other health behavior than could affect outcome.
- Establish patient goals and interventions.
- Ensures stage appropriate referrals for education, vascular access, and transplants are completed.
- Participates in patient enrollment in programs by gathering all clinical elements necessary.
- Educate patients about benefits of the programs.
- Performs medication reconciliations.
- Collaborates with several specialists on necessary care and services.
- Acts as a patient advocate, liaison, and information resource.
- Monitors and reports patient outcomes through plan of care.
- Provides chronic care education for nephrology and chronic / complex conditions.
- Provides culturally appropriate education.
