Overview
The Davies Project (TDP) is seeking a Bachelor's level Social Worker (BSW) to implement oral health (collaboratively with Delta Dental of Michigan) and other appropriate interventions and referrals as identified through a case management model of outreach, monitoring, and measurement and tracking of outcomes. TDP is a Lansing 501(c)3 nonprofit organization that provides transportation to medical appointments for children with long-term or serious health challenges, as well as expectant moms to prenatal appointments and parents to the NICU to bond with their newborns.
Responsibilities (examples do not include all tasks which may be required of the position and are subject to change at any time):
Coordinate activities for families within the agency, including but not limited to:
Handle initial intake of families to TDP. Ensure appropriate forms are complete, contact family by phone to explain TDP’s guidelines and gather further information (as needed), and enter family into TDP’s Assisted Rides tracking system.
The promotion of oral health for all family members.
Identifying, developing, and implementing a communication plan with TDP families.
Organizing and hosting occasional gatherings with families.
Facilitating, in conjunction with staff, an orientation process for families new to TDP.
Maintain up-to-date information on local nonprofit agencies to connect families with other valuable resources available to them. Maintain working relationships with these community agencies and partners.
Required
BSW with a thorough understanding of the case management model.
An understanding of the importance of oral health as it relates to overall physical health.
Respect for, sensitivity to, and understanding of cultural and socioeconomic differences.
Excellent interpersonal, written, and phone communication skills.
Strong organizational skills to connect with families and outside agencies.
Ability to serve as a liaison between families and other staff and outside agencies.
Creative approach to building stronger relationships with families.
Ability to work independently, troubleshoot, and conduct and complete assigned tasks in a timely manner.
Successfully pass a criminal background and sex offender check.
Valid driver’s license, current auto insurance, and safe driver’s record.
Preferred Candidate Has
Proficiency in Mac platform.
Skill set to learn Assisted Rides tracking system.
Available to work both in the office and remotely (on occasion).
