What are the responsibilities and job description for the Case Manager, Med HIV position at The Dimock Center?
Summary/Objective:
The HIV Medical Case Manager (MCM) provides medical case management services for clients with HIV disease. The MCM develops an on-going relationship with each client and is a liaison with Dimock departments and outside referral agencies, to continue services and to ensure that referrals are followed through. In addition, the MCM update providers of issues of medical case management including treatment adherence counseling to ensure continuity of care.
Essential Functions:
- Complete intake and assessment of clients per case management guidelines
- Accompany clients to appointments as necessary (i.e. medical, housing, court, financial)
- Develop and implement treatment plans.
- Develop a network of social service providers.
- Update Medical Case Management charts on a continuous basis.
- Monitor/update Excel spread sheet daily
- Monitor ECW for upcoming client appointment daily
- Perform patient reminder calls
- Medical Case Management chart audit review on entire case load quarterly
- Manage all related documentation relevant to client services.
- Facilitate support groups as deemed necessary
- Monitor patient CD4 and VL

Work Environment:
This job operates in an office environment, with some out of office client home visits as needed.
Physical Demands:
Duties may require standing and walking, or sitting for long periods of time.
Position Type/Expected Hours of Work:
Expected hours are Monday through Friday 9:00 am to 5:00 pm.
Required Education and Experience:
- Required: High School Diploma/GED
- Medical Case Management experience preferred
- Working knowledge of serving patients with HIV/AIDS and addiction.
- Experience working with culturally diverse communities.
- Familiarity with community-based agencies serving people of color.
- Good community and interpersonal skills.
- Bilingual/bicultural a plus.
