Transitional Home & Community Support ( THCS)
The purpose of a Transitional Home and Community Support (THCS)is to assist our traumatic brain injury customers to develop and/or relearn skills necessary to optimize independence and enhance the individuals quality of life while restoring the customer to an optimal level of physical, cognitive, and behavioral function within the context of the person, their family and community.

Duties & Responsibilities

• Identifying strategies that enable the customer to effectively compensate for cognitive and/or physical impairments that are barriers to the performance of the types of activities required for independent living and household management.
• Teaching strategies directly to the customer and family so that they can successfully manage roles.
• To keep and record documentation of all activities and events.
• Services are provided in the home and in the community.

Preferred Skills/Education:

• High School diploma or equivalent required
• Must be organized and possess time management skills
• Demonstrate relationship management, critical thinking, problem solving and decision making skills
• Basic computer skills and use of technology ( cell phones, faxing, copying, etc.)
• Good interpersonal, written and oral communication skills
• Ability to maintain confidentiality.

Qualifications and Requirements:

Valid Driver's License with reliable transportation ( your own car) and current car insurance

Certification in Adult First Aid and CPR or willingness to attend training required

Attend all mandatory trainings

Able to pass: reference checks, employment verifications, drug and background screenings

Regular access to the internet and a computer for daily documentation and submission

Current Locations:

The Kansas City, MO metro area including Blue Springs, Independence

Job Type: Full-time

Pay: From $18.00 per hour

Expected hours: 20 – 40 per week

Schedule:

• Day shift
• Evening shift
• Monday to Friday
• Weekends as needed

Shift availability:

• Day Shift (Preferred)

Ability to Commute:

Ability to Relocate:

• Kansas City, MO 64124: Relocate before starting work (Required)
