Qualifications: Must have a valid NC Driver License, and dependable transportation. Must be willing to transport Individuals in your personal vehicle. Must be willing to work one-on-one with Individuals with Intellectual and Developmental Disabilities. Must be willing to go into individuals homes to provide services.

JOB DUTIES:
- Must have the ability to work independently with minimal supervision.
- Perform medical monitoring and health care related tasks such as checking and recording routine vital signs as assigned.
- Document changes in behavior or health.
- Must provide personal care.
- Perform and assist with personal hygiene, grooming, feeding and ambulation as needed.
- Assist with diapering, toileting, including using changing tables, toileting chairs, Hoyer lifts and ceiling track lifts.
- Must be trained in medication administration.
- Must be G-tube trained.
- Must be able to lift 50-60 pounds on a daily basis.
- Must be able to perform physically demanding duties.
- Completes documentation within 24 hours of service delivery.
- Encourage self-help activities.
- Provides Transportation if needed.
- Reinforces positive behaviors and promotes social interaction.
- Monitors and documents progress of each assigned individual’s goals throughout the day.
- Helps develop new skills and interests.
- Work with team and communicates issues related to individual served.
- Ensure individuals safety and maintain a safe environment.
- Provides support and direction to achieve goals listed in individuals served Person-Centered plan.
- Maintains contact with individual served at all times, unless other arrangements have been made.
- Assists with program details to meet the needs and interests of individual members.
- Observes individual served to ensure their safety.
- Arrives on time, prepared to begin work with individual when their day begins.
- Otter duties as assigned by Supervisor.
