The Family Counseling Center of Fulton County, Inc. is seeking a compassionate and organized Targeted Case Manager to join our team. In this role, you will work closely with clients to develop personalized service plans, coordinate care, and connect clients with the necessary resources to support their recovery and well-being. You will collaborate with clients, their families, and a multidisciplinary team to ensure comprehensive and effective service delivery.
Key Responsibilities:
- Conduct thorough assessments and develop personalized service plans addressing clients' behavioral health, medical, and social needs.
- Coordinate access to services such as medical care, mental health treatment, housing, and community resources.
- Advocate for clients’ needs, ensuring their rights are respected throughout the process.
- Provide regular follow-ups with clients, monitor progress, and adjust service plans as needed.
- Respond to crises, providing immediate support and collaborating with other professionals when necessary.
- Maintain accurate case records, including assessments, service plans, and progress notes.
- Collaborate with clinical staff, social workers, and community organizations to ensure coordinated care.
- Participate in program development, quality improvement, and analysis of client feedback and outcomes.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Master’s preferred).
- Minimum 3 years of experience in case management or social work, preferably in a behavioral health or community health setting.
- Relevant certifications (e.g., Certified Case Manager) preferred.
- Strong knowledge of case management practices and community resources.
- Understanding of behavioral health and substance use disorder treatment, trauma-informed care, and recovery-oriented systems.
- Ability to manage a caseload, prioritize tasks, and work both independently and as part of a team.
- Strong communication, organizational, and problem-solving skills.
