The Family Place Utah is seeking a part-time (20 hours per week) Care Coordinator to join our Trauma Resiliency Project (TRP) in support of our mission, “Strengthening families and protecting children.” Must be bilingual Spanish and English and able to effectively interact with individuals from diverse cultures.

Duties Synopsis
Care Coordinators screen for trauma and need for services and connect families to services, then follow up and facilitate access to services. Care Coordinators also assist with outreach and educational activities throughout the community. Care Coordinators complete documentation, evaluation, data entry, and reporting activities as required for the TRP grant.
Qualifications
1. Bachelor’s Degree in a Human Services related background.
2. Ability to pass a criminal background check.
3. Proficient in English: verbal, reading, writing.
4. Bilingual Spanish/English required for some Care Coordinator positions.
5. Culturally sensitive, willing and effective at engaging positively with individuals from diverse cultures.
6. Punctual, thorough, caring, positive, and strong work ethic.
7. Ability to work as an effective team member.
8. Maintain client confidentiality and professionalism at all times.
9. Competent in computer use, i.e., databases, MS Word, Excel.
