Caregivers of New Jersey is an affiliate of The Family Resource Network. We are a statewide agency devoted to helping individuals with intellectual/ developmental disabilities and their families. We are currently seeking Support Coordinator candidates for our team at New Jersey Support Coordination.
General Description:
The Support Coordinator helps individuals with disabilities and their families to identify and link with needed services and supports to improve their quality of life within the home and the community.
As a Support Coordinator, you must be willing and able to travel and drive throughout the State of NJ as well as have a flexible schedule to accommodate consumers schedule.
Primary Job Functions:
- Develops individual plans of care, with input from the individual and family, which is based on the personal preferences and choices of the individuals being served;
- Utilizes available specialized and generic community resources in combination with other formal and informal supports and services;
- Maintains continuing contact with individuals and families as individual plans of care are put in place;
- All support coordination deliverables as outlined in the Policy & Procedure Manuals (SP & CCP)
- Assist individuals and their families in coordinating, selecting and overseeing their own supports;
- Works as part of a regional support team;
- Continues learning about relevant, local accessible services and providers;
- Informs providers and the Division about identified service gaps;
- Completes required trainings and attends meetings as assigned.
Qualifications/ Background:
- Bachelor’s Degree in a relevant discipline and 1-year equivalent experience working with adults (18 ) with intellectual or developmentally disabled population;
- Computer Proficient (MS Office Outlook, Word, Excel, PowerPoint);
- Able to solve problems independently, effectively and creatively;
- Excellent Organization and Communication skills;
- Excellent time management skills to be able to juggle multiple plans for multiple families simultaneously;
- Balance of work in the field and remote office- Must have a flexible schedule to meet the needs of our consumers;
- Valid Driver’s license and car to be able travel throughout the state of New Jersey
