We are looking for a Social Services professional to plan, organize, and develop the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be required, to assure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.
We are looking for a compassionate, dedicated individual who has a special interest in, and a positive attitude about, working with nursing home residents and the elderly. You will need the following:
- Certificate of Completion (Social Services Designee in Long Term Care)
- One year of social work experience in a health care setting, working directly with individuals.
- A minimum of one year experience in a management or director position.
- Must be able to communicate effectively.
- Creative problem-solving skills.
- Must be able to relate positively and favorably to residents and families and to work cooperatively with other employees.
General Duties and Responsibilit
Plan, develop, organize, implement, evaluate, and direct the social services programs. Participate in the community planning related to the interests of the facility and the services and needs of the resident and family. Participate in discharge planning, development and Implementation of social care plans and resident assessments. Initiate and participate in resident IDT meetings. Assist in arranging transportation to other facilities. Review, investigate and conclude departmental complaints and grievances from residents or facility personnel. Assist in QAPI sub-committees. Develop preliminary and comprehensive assessments of the social service needs of each resident. Develop a written plan of care of each resident. Ensure that social services components of the MDS are completed and signed off on a timely basis.
