The Case Manager is responsible for assuring quality service to all guests of the organization in a compassionate and professional manner. Overall, the role of a Case Manager is to support and empower our guests to achieve their goals, overcome challenges, and improve their quality of life by facilitating access to the appropriate services and resources.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Uphold the Mission, Vision and Values of The Gathering Inn and maintain a guest-first mentality at all times.
- Conduct comprehensive assessment of guests needs, strengths, and goals through initial intake. Based on this assessment, develop individualized care plans and goals in collaboration with the guests.
- Maintain accurate and up-to-date records of guest’s interactions, services provided, outcomes achieved, and any other required documentation in a timely, consistent manner. (Utilization of Homeless Management Information System)
- Coordinate and facilitate access to various service providers and resources to meet identified needs of the guests (Ensuring access, eligibility, and follow-through).
- Work collaboratively with community partners and resources for seamless delivery of services. This may involve scheduling appointments, arranging transportation, guest advocacy, and making referrals.
- Facilitate and lead appropriate assigned classes to guests in the program.
- Understand and maintain guest confidentiality and data privacy standards.
- Address any behavioral concerns that create disharmony, are against policy, or pose a safety risk in a calm, non-threatening manner.
- Participate in staff meetings, in-service training, and other relevant training (De-Escalation/Motivational Interviewing/CPR & First Aid, etc.)
QUALIFICATIONS/SKILLS:
- A bachelor's degree with one year experience or an equivalent blend of educational qualifications and practical experience is necessary. This could include a high school diploma and a minimum of five years of relevant experience. Alternatively, candidates with an associate's degree and three years of experience will also be considered.
- Knowledge of homelessness, poverty, mental health, substance use disorders, and trauma.
- Experience in working within shelter environments, low-barrier programming, or similar settings is a plus.
- Valid Driver License
