EXPERIENCE/JOB SKILLS REQUIREMENTS - High School Graduate
- CNA or Home Health Aide (HHA) course completion preferred
- Satisfactory completion of competency test
- Must be able to read, write, comprehend and carry out directions and instructions
- Able to function effectively with minimal direct supervision
- Good health status and emotional stability
- Demonstrate a strong commitment to resident service and service excellence
DUTIES AND RESPONSIBILITIES1. Can-Do Attitude- Demonstrates positivity, flexibility, and initiative in providing care. A Care Partner is expected to proactively address challenges, offer solutions, and maintain a supportive and encouraging approach toward residents and colleagues. • Adjusts to changing resident needs with patience and flexibility. • Maintains a positive attitude even in challenging situations. • Engages meaningfully with residents to enhance their well-being. • Creates meaningful engagement with residents (e.g., activities, conversation, emotional support). • Ensures residents’ rooms, clothing, and personal spaces are clean and well-maintained. • Enhances the dining experience by assisting with meals in a respectful, dignified manner.2. Accountability for Actions and Decisions- The extent to which a Care Partner efficiently completes tasks and provides high-quality care in a specified period. This includes the ability to work independently, manage responsibilities, and collaborate effectively with team members to ensure residents receive timely and compassionate care. •. Performs assigned tasks efficiently and within expected timeframes. • Follows through on responsibilities without repeated reminders. • Adheres to attendance policy - works all scheduled shifts or follows proper procedures when unable to work. • Arrives on time and ready to begin shift responsibilities in uniform and with name tag. • Takes ownership of tasks and ensures responsibilities are completed thoroughly.3. Respectful Collaboration - Fosters a culture of teamwork, open communication, and mutual respect with residents, families, and fellow Care Partners. Education through Continuous Training - A commitment to ongoing learning and professional development. Care Partners are expected to complete required training, stay informed about best practices, and apply new knowledge to improve resident care and team efficiency. • Understands what has been taught and applies job responsibilities effectively. • Completes required trainings on time. (RTasks – Care Fundamentals and Struther’s and in person training) • Attends and participates in staff/house meetings (90% attendance or better).5. S – Safety is Always a Priority - Ensures that residents, families, and team members feel secure in their environment. Care Partners follow all safety protocols, medication guidelines, and emergency procedures, maintaining a clean, organized, and hazard-free home to protect those in their care. • Follows care plans accurately. • Administers medications safely and as delegated. • Adheres to policies and procedures, including emergency and infection control protocols.
