Position Summary: The Clinical Coordinator supports the clinical team and coordinating resident services. Responsibilities include, but are not limited to, coaching and training team members, scheduling and timekeeping, assisting with performance management and evaluations.

Experience and Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required.
· 2 years of experience preferred in assisted living, long-term care, or hospital industry; or experience/exposure working with senior population
· Ability to handle multiple priorities
· Ability to delegate assignments to the appropriate individuals based on their skills, roles, and interest
· Ability to make choices, decisions, and act in the resident’s best interest
· Possess written and verbal skills for effective communication and the ability to facilitate small group presentations
· Demonstrates good judgment, problem-solving, and decision-making skills
· Demonstrates proficiency in computer skills such as Google applications
· Maintains universal care precautions of cleanliness, hygiene, and health standards
· Ability to work flexible hours as needed and to be available for residents and family members when needed
· Communicates regularly with families and physicians as appropriate
Essential Duties
· Supporting leadership of Care staff; timekeeping; assist with performance appraisals; scheduling the staff, communicate regularly with Supervisor/Administrator.
· Reviews, reads and notates daily logs to document and learn about pertinent information and any resident’s physical and/or behavioral changes
· Assists with resident assessment process to determine levels of care with appropriate staffing levels
· Prior to move-in, reviews resident’s file and meets with family to discuss resident profile; coordinates healthcare needs with Administrator.
· Maintains clinical documents, ensuring that documents are current and up to date
· Coordinates resident services
· Maintains resident medical records
· Helps maintain the self-respect, personal dignity, and physical safety of each resident adhering to the standards of Resident Rights
· Understand the recognition of resident changes in condition and takes appropriate action
· Ensures established safety procedures are being followed at all times
· Maintains universal care precautions of cleanliness, hygiene, and health standards
