Job Summary :
We are seeking a Care Manager Supervisor to oversee a team of Care Managers responsible for providing care management services to our members. The successful candidate will have experience in care management, supervision, and leadership. The Care Manager Supervisor will be responsible for ensuring that care management services are delivered in accordance with our organization's policies and procedures, as well as state and federal regulations.
Responsibilities :
- Supervise a team of Care Managers, providing guidance, support, and oversight to ensure that care management services are delivered effectively and efficiently.
- Conduct regular meetings with Care Managers to discuss member care plans, progress, and challenges.
- Review and approve care plans, ensuring that they are comprehensive, individualized, and aligned with our organization's policies and procedures.
- Monitor and evaluate the quality of care management services, identifying areas for improvement and implementing corrective actions as needed.
- Collaborate with other departments, including clinical, behavioral health, and social services, to ensure comprehensive care coordination.
- Develop and maintain relationships with community-based providers, hospitals, and other healthcare organizations to facilitate care coordination and transitions.
- Participate in quality improvement initiatives, including data analysis, performance metrics, and outcome measurement.
- Ensure compliance with state and federal regulations, including HIPAA and Medicaid guidelines.
