APPLICANT REQUIREMENTS:
Bachelor level degree in the Social Work field with a minimum of 3 years’ experience working directly with at-risk families.
This position is Monday through Friday, no late nights or weekends.
Must have valid Florida driver's license, current auto insurance and be insurable by our current carrier.
JOB RESPONSIBILITIES:
· The Case Management/C.A.R.E.S. Coordinator provides in-home services to families at high risk for entering the child welfare system to include education, safety planning, and assessment.
· Using a strength based approach the Coordinator builds, and facilitates, a care team comprised of family, community and professional supports to assist the family in carrying out the goals written in the treatment plan.
- Position based in Volusia County, will be covering Volusia and possibly Flagler Counties. Mileage reimbursement at higher than the State average.
