What are the responsibilities and job description for the 29-I Operations Liaison position at The House of The Good Shepherd?
Basic Function/Overall Responsibilities: Responsible for assisting with Medicaid Managed Care coordination for the 29-I VFCA Health Facility, while assuring and monitoring healthcare services for our youth in care. The 29-I Liaison is specifically connected to the Utica and Watertown foster care programs, with a strong focus on discharge healthcare planning.
Principal Assigned Responsibilities:
- Serves as a point of contact for Medicaid Managed Care Plans (MMCP) to assist with: discharge planning, insurance card requests/replacements, locating a service provider within network, and requesting Single Case Agreements as needed.
- Ensures medical and clinical staff are properly supported in the areas of service providers for children in care, medical record requests, and checking insurance coverage status prior to scheduled appointments.
- Upload MMCP notifications and relay information as needed to the medical and Foster Care team.
- Attend service plan reviews and/or treatment team meetings as needed in relation to discharge planning documentation.
- Accurately document, track, and share discharge plans as required per the 29-I guidelines.
- Maintain effective business relationships with the medical and/or clinical provider network on behalf of the organization.
- Support standardized practice of medical and clinical services across the agency.
- Positively represent the agency during OCFS/DOH quarterly calls and other professional meetings.
- Adheres to agency policies and procedures; follow and support agency mission, vision and values including participating in internal survey and data collection for continuous improvement initiatives.
- Other tasks as requested by supervisor.
Education and Experience Requirements:
- Bachelors Degree in behavioral health, health care policy, administration or management required
- Experience working in the medical, behavioral health, child welfare and/or public health insurance fields with 2 years of experience preferred.
