Program Summary
The Community Works Project (CWP) is a 3-agency collaborative project providing job preparation and placement services to participants in the program of the ODHS Temporary Assistance to Need Families (TANF) who are participating in Job Opportunities Basic Skills (JOBS) program. CWP serves seven Oregon Department of Human Services (ODHS) locations.
Position Summary
The Behavior Health Consultant (Mental Health / Alcohol & Drug) position is expected to serve seven ODHS locations during regular ODHS hours of business (with flexibility to meet and respond to participants’ needs outside those hours/locations on an exceptional basis). The Behavioral Health Consultant is responsible for serving participants referred by Oregon Department of Human Services (ODHS) Family Coaches for assessment or Mental Health and/or Alcohol & Drug services. The position assesses clients’ needs, determines appropriate types and level of services, and implements culturally responsive and trauma informed interventions for specific cases. This is direct clinical service role providing transitional therapeutic support to clients to establish rapport and engagement, to find participants community providers who accept Medicaid, and on-going case management and follow-up to referrals to ensure participants are receiving the services they need.
Essential Functions
- Conduct screenings/evaluations with participants to determine current need for mental health services and/or drug and alcohol counseling services based on histories of mental, emotional, and physical disabilities; trauma; alcohol and other drug use, past/current mental health services; family, social and work relationships; and any other information disclosed by the participant
- Create a care plan based on the assessment with measurable care goals. Monitor and document ongoing progress toward these goals.
- Establish referral system for culturally responsive and culturally specific mental health and drug & alcohol services with local providers
- Help participants problem-solve to identify different JOBS program activities which accommodate their limitations or disabilities and align with their goals
- Lead care planning through case management interventions and communication with all parties, including participant, to reach desired goals and objectives. Conduct home and community visits when needed.
- Maintain ongoing contact with service providers and request updated information at those times specified by the service providers. Advocate for participant when necessary.
- Obtain, verify, and interpret information from service providers and track participants’ attendance and/or assist Family coaches in doing this. Enter all received documentation, participant/provider contacts and attendance in TRACS and Apricot database systems
- Maintain close coordination with Family Coaches and Medical Case Consultant and Health Navigation Specialist on participants’ progress, advocating for participant when necessary
- Work collaboratively with CWP Medical Case Consultant, Health Navigation Specialist, CWP Program Manager, and ODHS Leadership to streamline Mental Health and Alcohol and other Drug services and processes and co-create ways for medical and mental health teams to work together to serve participants with multiple barriers
- Work to improve participant access to services, resolve problems that may interfere with participants’ engagement in JOBS services, and identify/avoid duplication of services
- Make referrals to the pre-SSI program, Vocational Rehabilitation, medical providers, and community health resources as appropriate
- Provide strength-based assessments using assertive engagement/motivational interviewing in direct consultation with participants
- Submit monthly reports on a strict timeline and contribute to other reports as needed
- Perform short-term crisis intervention for participants, as necessary
- Regularly attend meetings with other CWP teams and DHS to discuss operational issues and intensive case needs
Secondary Functions
- Participate in staff meetings, trainings, committees, and volunteer activities to support CWP and agency’s goals
- Other duties and responsibilities may be assigned
Education & Experience
MINIMUM JOB SPECIFIC QUALIFICATIONS:
- Extensive knowledge and experience working with communities of color such as: African
- Americans, Latinx, Native Americans, immigrants and refugees. Bilingual or bicultural persons are strongly encouraged to apply
- Experience working with participants with alcohol and other drug addiction required
- Must be able to maintain and adhere to the highest ethical standards of the social work/counseling profession by maintaining strict confidentiality, striving for the highest quality level of client care and staff alike, and follow mandatory reporting duties as specified by the Oregon Law 419B.010
- Familiarity with local non-profit resources to support emotional well-being and family stability
PREFERRED QUALIFICATIONS:
- Master’s in Social Work, Psychology, Counseling, or related field 2 years’ experience
- Bachelor’s in Social work, Psychology, Counseling, or related field 5 years’ experience
