Job Description
This position involves working in a team approach to promote timely access to appropriate care and increase the utilization of preventative care. Responsibilities include reducing emergency room utilizations and hospital readmissions, and facilitating patient access to appropriate medical and specialty providers.
Qualifications
- Must have 2 years of NP experience
- Colorado licensed NP with experience working in a clinic setting
- Current unrestricted DEA License
- Bilingual skills (English/Spanish) desirable
