Join TheKey’s Geriatric Care Manager group (formerly Advocare) is hiring an Intake Client Services Coordinator. As an Intake Client Services Coordinator for Care Management services, you will build relationships with potential clients, coordinate the intake process of assessments, and ensure overall client satisfaction with our services.
What You'll Do:
- Responding to prospective client inquiries and ensuring they receive the information they need to move forward with our services.
- Coordinating a successful assessment and quality service responses for potential clients with our Care Management staff is crucial.
- Coordinating assessments including identifying and creating visits for assessment with our Care Management staff in collaboration with the clinical director.
- Track activity effectively and complete all steps of successful intake and follow up with assigned Geriatric Care Manager to ensure all intakes completed and the follow-up status.
- Track referral patterns and build trusting relationships with referral sources and their office staff.
- Participate in community awareness events, marketing, and networking activities.
- Communicating effectively within the local care management team to ensure that our care plans meet the clients’ needs, that stakeholders and referral sources are satisfied, and that relevant information is communicated in an effective and timely manner
- Developing strong relationships with the Care Management team to ensure a strong working relationship, effective management of clients and seamless communication.
The Ideal Candidate:
- Bachelor’s Degree in a related Human Services Field is preferred
- Previous experience in a Healthcare, Social Work or Homecare environment
- Excellent written and verbal communication skills; including, active listening
- Ability to assess callers concerns and communicate how Care Management can assist with current or long-term solutions
- Ability to overcome objections and creative problem solver
- Ability to make and take a volume of phone calls as well as email
- Time management, organization, follow-up, and multi-task skills
