Help Wanted: Case Manager/Community Resource Specialist
Position: Case Manager (Part-Time/Flexible 20-40 hrs)
Program: Home and Community Support Services for Individuals with Brain Injury
Location: Springfield and surrounding areas (up to one hour away)
Salary Range: $19

Job Description: We are seeking caring and dedicated Direct Care Workers to provide home and community support services to individuals with brain injuries. This position is available as PRN, PART TIME, and working in to FULL TIME, offering flexibility to meet your needs.
Activities Included in This Services
- Identifying strategies to help participants compensate for cognitive and/or physical impairments that are barriers to independent living and household management. Examples include memory notebooks, systematic calendar notes, alarm watches, timers, tape recorders, adaptive writing instruments, etc.
- Providing instruction and training in the acquisition of strategies and skills the participant needs to care for personal needs independently, and to plan, organize, and carry out activities suitable for their lifestyle and family role. Examples include directing personal care, performing household chores, menu planning, grocery shopping, meal preparation, budgeting, auto/lawn care, creating and maintaining a weekly schedule, developing emergency contingency plans, arranging and accessing public transportation, and scheduling and keeping appointments with social service agencies, attorneys, physicians, etc.
- NAVIGTE COMMUNITY RESOURCES
- Conducting training in home and community settings.
- Assisting families in adjusting to changes in roles by providing direct training in techniques, suggesting alternative solutions to common problems, identifying natural supports, and referring family members to appropriate services.
- Transport clients in your own vehicle as needed for appointments and community activities.
- Monitor and report on client progress and well-being.
- Maintain accurate and timely documentation of services provided.
Requirements:
- Knowledge of community resources.
- High school diploma or equivalent required; Bachelor's degree preferred.
- Compassionate and empathetic attitude with a strong desire to help others.
- Excellent communication and interpersonal skills.
- Reliable transportation and ability to transport clients in your own vehicle.
- Flexibility to work various shifts, including evenings and weekends, as needed.
