Home Health Agency located in Sherman Oaks, CA looking for an experienced LVN Case Manager.
LVN Case Manager will oversee clinical and administrative operations, manage case files, and hire and train new employees. Other duties and responsibilities of Case Managers may include:
- Manage patient care plans from the office, coordinating services for home health patients.
- Work with physicians, patients, and caregivers to ensure that care plans are implemented effectively.
- Perform clinical assessments, review patient information, and communicate necessary updates to the home health team.
- Oversee the coordination of medical equipment, supplies, and home health services for patients.
- Monitor patient progress and make adjustments to care plans based on patient needs.
- Document all patient interactions and care provided, ensuring compliance with company policies and state regulations.
- Provide clinical guidance and support to home health staff and caregivers.
- Ensure proper utilization of resources to promote effective and efficient care delivery.
Qualifications:
- Current LVN license in California.
- Minimum of 1-2 years of experience in home health care, case management, or office-based healthcare coordination.
- Strong clinical knowledge and case management skills.
- Excellent organizational, communication, and time-management abilities.
- Experience working with home health systems and electronic medical records (EMR) is a plus.
- Ability to work independently and collaborate with a multidisciplinary team.
