We are seeking a highly skilled MDS Coordinator to join our team at The Laurels of Athens Careers. As an MDS Coordinator, you will be responsible for providing oversight of the RAI process, conducting assessments and care plan coordination, and coordinating scheduling the RAI process with the interdisciplinary team.
You will assess residents through physical assessment, interview, and chart review, and discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff. You will also review information from hospitals, consults, and outside agencies and use such information in the completion of the assessment and care planning.
