Job Summary
The MDS Coordinator provides oversight of the RAI process and conducts assessments and care plan coordination for our guests. This role requires strong clinical skills, attention to detail, and excellent communication skills.
Responsibilities:
- Completes the MDS, CAA's, and care plans within regulated time frames.
- Coordinates scheduling the RAI process with the interdisciplinary team.
- Assesses residents through physical assessment, interview, and chart review.
- Discusses resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure.
- AANC certification a plus.
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS).
