We are seeking an organized and efficient MDS Nurse to join our team. This role involves remaining clinically involved in patient care without being a bedside nurse, allowing for a more autonomous work environment.

Key Responsibilities:
The MDS Nurse coordinates assessments and care plans for residents assigned to their care, ensuring timely completion of tasks such as:
- MDS, CAA's, and care plan completion within regulated time frames.
- Resident assessments through physical evaluations, interviews, and chart reviews.
- Collaboration with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff, to discuss resident care needs.
- Review of information from hospitals, consultants, and outside agencies to inform assessment and care planning processes.
- Coordinate and initiate significant changes to MDS.
- Prepare for utilization review meetings, maintaining MDS assessments per Medicare schedule and skilled board monitoring for Medicare days and PDPM utilization in the absence of the Care Management Coordinator.
- Stay current with AANAC requirements.
Requirements:
- Registered Nurse, RN or Licensed Practical Nurse with experience may be considered.
- AANC certification is a plus, RAC-CT.
- Knowledge of the Resident Assessment Instrument (RAI) process, including principles of Prospective Payment Process (PPS), strongly preferred.
- Experience as an MDS Nurse.
