About the Role
The Laurels of Bedford seeks a skilled Resident Assessment Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for conducting thorough resident assessments, developing care plans, and coordinating the RAI process.
Key Responsibilities
- Assess residents' physical and emotional needs, and develop personalized care plans in collaboration with the healthcare team.
- Manage the RAI process, ensuring timely completion of MDS, CAA's, and care plans.
- Communicate effectively with healthcare professionals, families, and residents to ensure high-quality care and services.
Requirements
- A registered nurse (RN) or licensed practical nurse (LPN) with a strong background in nursing and assessment coordination.
- Experience as an MDS Nurse or in a similar role is highly preferred.
- Knowledge of the RAI process and PPS principles is essential.
Compensation and Benefits
