About the Role:
The Laurels of Kent Careers is seeking a skilled nurse to fill the position of MDS Nurse. This role involves working with residents who require rehabilitation and skilled nursing services, providing them with the necessary care and support to achieve their goals.
Key Responsibilities:
- Conduct thorough assessments of resident needs through physical evaluation, interviews, and chart review
- Develop and implement individualized care plans that meet the unique needs of each resident
- Collaborate with interdisciplinary teams, including physicians, nurses, social workers, therapists, and dietary staff, to ensure comprehensive care
- Stay up-to-date with American Association of Nursing Assessment Coordinators (AANAC) requirements and best practices
- Manage PPS meetings and maintain accurate records of Medicare days and RUGs utilization
Requirements:
- Registered Nurse (RN) license
- Experience as an MDS Nurse in skilled nursing and rehabilitation settings
- Familiarity with the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS)
